Online Application for Doctor of Medicine Applicants

- Fill out the Online Application Form and follow the instructions given (choose
“cross enrollee”).
- Log in to the applicant’s portal and choose your medical examination and uniform measurement schedules.
- Submit the following requirements to the DLSU Manila College of Science to receive your Enrollment Permit and Enrollment Procedure in your applicant’s portal.
Requirements:
- Photocopy of Birth Certificate
- 1 pc. 2×2 picture (white background)
Additional requirements for Foreign Medicine applicants
4. Proceed with enrollment.
(All foreign students are required to pay the $7,000 foreign fee upon enrollment).
