ENTRANCE SCHOLARSHIP
This is given to incoming DLSMHSI undergraduate students who graduated with High/Highest Honors from Senior High School, in a graduating batch of at least 50 students.
Pre-requisites:
- Newly admitted regular student
- Filipino citizen and resident of the Philippines
- Graduated With High/Highest Honors
- Belongs to:
a. Top 20 examinees of the DLSMHSI Entrance Scholarship Qualifying Exam – for incoming Nursing and Medical Laboratory Science students
b. Top 10 examinees of the DLSMHSI Entrance Scholarship Qualifying Exam – for other undergraduate programs offered

Application Procedures:
- Fill out the Scholarship Application Form for Entrance Scholarship-UG.
- Fill out and upload the following requirements in PDF format on the assigned Google Drive. Please rename the files as: Last Name, First Name, Middle Name and Temporary/Student ID number.
Requirements:
- Completed and signed Entrance Scholarship Recommendation Form by the School Principal or Director, certifying the student’s status as With Highest Honors or With High Honors in a graduating batch of at least 50 students.

ACADEMIC SCHOLARSHIP
This scholarship is granted to DLSMHSI undergraduate students who have demonstrated consistent academic excellence during their stay at the Institution, in accordance with the grade requirements set by their respective College.
Pre-requisites:
- Regular student
- Filipino citizen and resident of the Philippines
- With grades not lower than 85 in all subjects
- GWA of 92% or higher / Has met the grade requirement set by College

Tuition Fee Discount
30%-100% (depending on the existing College policy)
Financial Scholarship
This is given to incoming DLSMHSI undergraduate students who are in need of financial assistance. Applicants are screened and evaluated based on the requirements submitted and through interview or house visitation by the Scholarship Committee. Approved Financial Scholars are required to render 50-100 hours of service as Office Assistant (depending on the tuition fee discount granted) as their return service.
Pre-requisites:
- Regular student
- Filipino citizen and resident of the Philippines
- Has passed the background check and interview of the Senior High Scholarship Committee
- Minimum grade requirement of 83 per subject (depending on the existing College policy)
- General Weighted Average (GWA) of 87 or higher (depending on the existing College policy)

Application Procedures:

- Fill out and upload the following requirements in PDF format on the assigned Google Drive. Please rename the files as: Last Name, First Name, Middle Name and Temporary/Student ID number.
Requirements:
- Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance

- Preferably with a score of 160 above in the DLSMHSI College Admissions Test (CAT)

SERVICE SCHOLARSHIP
This is given to officers of the DLSMHSI Institutional Student Council.

Callejas-Mijares Scholarship Award
This is given to incoming DLSMHSI BS Biochemistry students who are in high need of financial assistance. Applicants are screened and evaluated based on the requirements submitted and through interview or house visitation by the Scholarship Committee. Approved scholars are required to render service by working at Jefcor Laboratories, Inc. equivalent to one (1) year for every year of integrated curriculum scholarship.
Pre-requisites:
- Newly admitted or currently enrolled regular BS Biochemistry student (must be in the Professional Track)
- Filipino citizen and resident of the Philippines
- Of good moral standing
- No failing marks in any academic or non-academic subject
- No grade lower than 80% in professional subjects
- Must maintain a General Weighted Average (GWA) of at least 85% per term

Tuition Fee Discount
Full coverage of tuition, miscellaneous fees, special assessments, and other institutional charges, including book and uniform allowances, as well as required physical and medical examinations
City Government-Recommended Scholarship
This is automatically given to incoming DLSMHSI undergraduate students who are recommended by the Office of the Mayor of the City of Dasmarinas.
Pre-requisites:
- Newly admitted regular student
- Filipino citizen and resident of the Philippines
- Recommended by the Office of the Mayor of the City of Dasmarinas

Application Procedures:
1. Fill out the Scholarship Application Form for SEP Scholarship-UG.
2. Fill out and upload the following requirements in PDF format on the assigned Google Drive. Please rename the files as: Last Name, First Name, Middle Name and Temporary/Student ID number.
Requirements:
- Certificate of Scholarship issued by the Office of the Mayor of the City of Dasmarinas.
- Certificate of Moral Character

Tuition Fee Discount
100% total fees (50% subsidized by the City Government of Dasmarinas and 50% subsidized by the Institution)
DLSMHSI Scholarship to Employment Program
- This shall be offered to qualified graduating students who may be interested to avail of the full scholarship opportunities provided by DLSMHSI (first and second semesters), with a 2-year employment opportunity at the De La Salle University Medical Center (DLSUMC) immediately after passing the licensure examinations.
- This also covers full subsidy on the licensure examination reviews (review fees, application fees, board and lodging, and all other related expenses).
- This program shall initially run for 3 school years, then to be assessed after, depending on the turnout of applicants at the DLSUMC and its manpower needs.
NOTE: Dependents of DLSMHSI employees who have started to avail of the Enhanced Brother President Scholarship Program may be allowed to apply for the said scholarship during their fourth year subject to the screening process and policies and guidelines set for this scholarship program.
THRUST: From Scholarship to Employment
Pre-requisites:
1. Filipino citizen residing in the Philippines
2. 4th year or incoming 4th year student of any of the following programs:
a. BS Nursing
b. BS Pharmacy
c. BS Medical Laboratory Science
d. BS Physical Therapy or BS Occupational Therapy
e. BS Radiologic Technology
3. Willing to sign the contract for the DLSMHSI Scholarship to Employment Program (see the Memorandum of Agreement)

Application Procedures:
1. Fill out the Scholarship Application Form for SEP Scholarship-UG.
2. Fill out and upload the following requirements in PDF format on the assigned Google Drive. Please rename the files as: Last Name, First Name, Middle Name and Temporary/Student ID number.
Requirements:
- Letter of Intent addressed to the Vice Chancellor for Academics
- Curriculum Vitae
- Certificate of Moral Character (Email [email protected] to request.)
- Copy of the Certificate of Grades from Year Level I to Year level III issued by The Registrar
- Parent’s Consent Form
- Billing statement (electricity, water, telephone, internet, cable, etc.) for the last three months (must submit all)
- Vicinity map/sketch of the house’s location
- Most recent Income Tax Return (ITR) of parents / Affidavit of Non-Filing of Income Tax Return (ITR) / Barangay Captain or Mayor’s certification of family and income potential / Certificate coming from Barangay if both parents are not employed or only one is employed
- Certificate of Employment of both parents (if applicable)
- 1 pc. 2×2 colored picture (white background)
Note: Please submit a hard copy of all the above requirements to The Registrar – Admissions, Scholarship, and Testing Section (TRASTS).

Screening Process:
- All interested applicants shall submit all the application requirements to TRASTS.
- The documents submitted shall be forwarded by TRASTS to the Scholarship Committee of the college concerned.
- All applicants are required to attend the interview to be conducted by the Scholarship Committee Chair of their Colleges on the date and time given to the applicants through email.
- After evaluation, the Scholarship Committee shall submit its final recommendation to the Office of the Vice Chancellor for Academics for approval.
- Applicants recommended by their college will be endorsed by TRASTS to the Hospital Division for a Final Interview. The exact date and time of this interview will also be communicated to applicants.
- The Office of the Vice Chancellor for Academics, through TRASTS, will release the final results and notify all successful applicants.
Scholarship Privileges
- Full coverage of tuition, miscellaneous, and other school fees for 4th year (1st and 2nd semesters), excluding graduation fee, graduation pictorial fee, and any other special collections mandated by the College or Institution.
- Php15,000.00 cash allowance for review in outsourced centers, which includes review materials for the Philippine Boards or free review if enrolled in the Institution’s in-house or accredited review center; Php30,000.00 for board, lodging, and utilities, provided that the accommodation is near the review center; and In-house transportation to and from the exam site.
NOTE: The scholars may choose their preferred review centers, as long as they are accredited and recognized by their respective colleges.
Employment Opportunities
- Two-year employment contract at DLSUMC, with entitlement to prevailing salaries, benefits, and other hospital-provided packages.
NOTE: After completing the return service agreement, DLSUMC and the DLSMHSI Human Resource Management Department will provide a strong and well-crafted employment certification and endorsement if the scholar considers overseas employment.
Externally Funded Scholarships
The requirements and policies for this type of scholarship are determined by the sponsoring organizations and evaluated by the appropriate offices. However, applicants are required to file their applications through The Registrar – Admissions, Scholarships, and Testing Section (TRASTS).

President Scholarship Program (PSP)
These are scholarship benefits given to the children of regular DLSMHSI employees who wish to enroll in DLSMHSI and/or DLSU-Dasmarinas.

Prerequisite:
Scholar’s parent (‘applicant’) must be a regular employee of DLSMHSI
Requirements:
- Application Form
- Original Certificate of Employment of the applicant (faculty or staff), issued by Human Resource Management, certifying that the applicant has been granted permanent status
- Clear copy of the Birth Certificate of the applicant’s dependent
- Adoption papers if applicant’s dependent is legally adopted
- Prospectus of the course enrolled in
- Certificate of Admission from the school or course enrolled in
- One (1) 2×2 color picture (white background with name tag)

Significant Details / Information Relatable to Scholarships
- All scholar-applicants are required to submit their letter of intent addressed to their Collegiate Scholarship Committee Chairs, fill-out the Scholarship Application Form, and submit the requirements stipulated on or before the set deadline by TRASTS. Old grantees may only submit additional information for updating purposes.
- All applications shall be screened by TRASTS before forwarding the documents to the colleges concerned through the Collegiate Scholarship Committee chairs and College Deans / Directors. The college, through the Scholarship Committee, then evaluates the documents (the Scholarship Committee Chair may opt to call for an interview with the applicant as the application process continues) and submits a list of prioritized candidates for scholarships to the TRASTS. The types of scholarships to be awarded depend on what is/are being applied for by the scholar applicants. However, TRASTS, through the Scholarship Committee, may award a different type of scholarship if the applicants have been disqualified from their desired type and available slots from other types of scholarships have been identified and the applicants themselves have satisfied all of the requirements. TRASTS may also recommend to the President’s Scholarship Fund if slots are still available and the applicants satisfy the same requirements.
- All prospective scholars under the fund assistantship of the President through individual sponsorships shall also be pre-screened by TRASTS. Upon recommendation of TRASTS (submission of the list of qualified applicants), the President shall choose the applicants who shall be granted scholarships.
- All scholar-applicants must wait for the release of the final evaluation report by TRASTS, the Vice Chancellor for Academics, and the Chief Finance Officer in the case of the BPSP, EBPSP, EDSP / GFCSP, and SLMEB scholar-applicants.. Likewise, they are advised to wait for further announcements through the TRASTS Bulletin of Information, DLSMHSI website, and other correspondences. The final list of scholars shall be posted in the DLSMHSI TRASTS webpage. The scholars can also visit the TRASTS office to confirm their status.
- All undergraduate scholars are enjoined to renew/re-apply for their scholarships every semester including the PSP.
- All scholars must fill out and submit the Memorandum of Agreement (MOA) at the beginning of every school year. The MOA contains all the policies and guidelines, conditions of scholarships, the provisions for the mandatory return of service, etc.
- TRASTS prepares the consolidated list of approved scholars and their completed and signed MOA to be submitted to the Vice Chancellor for Academics and the Chief Finance Officer for final approval.
Grounds for Denial of Scholarship Application
- Failure to comply with scholarship policies
- Any violations against the Memorandum of Agreement
- Deficiency in requirements
- Falsification of documents
- Commission of major offense from The Student Affairs of DLSMHSI, college of origin, or community
- Unavailability of slots and/or budget for the scholarship applied for
