SCHOLARSHIP APPLICATION FOR DLsMHSI EMPLOYEES
The President Scholarship Program (BPSP) / Enhanced Brother President Scholarship Program (EBPSP) / Employees’ Dependents (EDSP) / Grand Father Clause (GFCSP) Scholarship Program are scholarship benefits given to the children of DLSMHSI regular employees who wish to enroll at DLSMHSI and DLSU-Dasmarinas (DLSU-D).
Procedure for New Scholarship Applications
Prerequisite:
Regular employee of De La Salle Medical and Health Sciences Institute
1. Fill out the Scholarship Application Form for DLSMHSI Employee (New).
2. Download and accomplish the following fillable Scholarship Forms:
a. Scholarship Application Form (to be filled out by the student)
i. Scholarship Application Form (to be filled out by the student)
b. Employee Scholarship Application Form
- Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)
- Enhanced Brother President Scholarship Program (EBPSP)
- Romeo P. Ariniego, MD Scholarship Grant (RPAMDSG)
- St. La Salle Medical Education Benefit (SLSMEB)
d. Authorization Letter
- Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)
- Enhanced Brother President Scholarship Program (EBPSP)
- President Scholarship Program
- St. La Salle Medical Education Benefit (SLMEB)
3. Fill out and upload the following requirements together with the completed and signed Scholarship Forms in PDF format on the assigned Google Drive. Please rename the files as: Last Name, First Name, Middle Name & Temporary/Student ID number.
Requirements:
- PSA Birth Certificate of employee and student
- Adoption papers if dependent is legally adopted
- Certificate of admission in the school or course enrolled in
- Prospectus of the course enrolled in
- Employment Certificate
4. Please read the reminder shown on the last part of the Google form per type of scholarship.
Procedure for Scholarship Renewal
- Fill out the Scholarship Application Form for DLSMHSI Employee (Renew).
- Download and accomplish the following fillable Scholarship Forms:
- Scholarship Application Form (to be filled out by the student)
- Employee Scholarship Renewal Form
- Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)

- Memorandum of Agreement (MOA)
- Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)
- Enhanced Brother President Scholarship Program (EBPSP)
- Romeo P. Ariniego, MD Scholarship Grant (RPAMDSG)
