
SCHOLARSHIP APPLICATION FOR DLsMHSI EMPLOYEES
The President Scholarship Program (PSP) / Employee's Dependent Scholarship Program (EDSP) / Grand Father Clause (GFC) / St. La Salle Medical Education Benefit (SLMEB) / Romeo P. Ariniego, MD Scholarship Grant (RPAMDSG) are scholarship benefits given to the children of DLSMHSI regular employees who wish to enroll at DLSMHSI and DLSU-Dasmarinas (DLSU-D).
President Scholarship Program (PSP)

Prerequisite:
Regular/permanent employee/permanent full-time faculty of De La Salle Medical and Health Sciences Institute
Application Procedures (for new applicants):
1. Prepare the following requirements:
a.
Scholarship Tuition Fee Discount Authorization Form (to be signed and verified by HRD).
b. Prospectus of the course enrolled in (for dependents who will enroll at DLSU-D)
c. A clear copy of the DLSMHSI employee's birth certificate and their dependent / adoption paper if the dependent is legally adopted.
d. Certificate of admission to the school of the course enrolled in (for dependents who will enroll at DLSU-D)
e. Copy of the Latest Tuition Fees of the Program (for dependents who will enroll at DLSU-D)
2 . Download or secure the following forms from Center for Lasallian Admission, Scholarship and Testing (CLAST)
a. Application Form President Scholarship Program (PSP)
b. Scholarship Application Form (to be filled out by the dependent)
c. Memorandum of Agreement for PSP
d. Authorization Letter
i. For DLSMHSI PSP Enrollment
ii. For DLSU-D PSP Enrollment
3. Submit the completed and signed forms and requirements to Center for Lasallian Admission, Scholarship and Testing (CLAST) every Monday-Friday, 8:00 am – 5:00 pm, at least two (2) weeks before the enrollment period.
4.
Secure the Signed Authorization Letter to CLAST at least one (1) working day upon submission. The DLSMHSI Employee should also provide a receiving copy of the Signed Authorization Letter after they enroll their dependent.
Application Procedures (for renewal applicants):
1. Prepare the following requirements:
a. Registrar-certified true copy of grades earned during the previous semester
b. Proof of re-enrolment in the subject/s failed, if any
c. Proof of payment for the re-enrolled subject/s failed, if any
2. Download or secure the following forms from Center for Lasallian Admission, Scholarship and Testing (CLAST)
a. Application Renewal Form President Scholarship Program (EDSP/GFC)
b. Memorandum of Agreement for EDSP/GFC (to be submitted during 1st semester only)
c. Authorization Letter
i. For DLSMHSI EDSP/GFC Enrollment
ii. For DLSU-D EDSP/GFC Enrollment
3. Submit the completed and signed forms and requirements to Center for Lasallian Admission, Scholarship and Testing (CLAST) every Monday-Friday, 8:00 am – 5:00 pm, at
least two (2) weeks before the enrollment period.
4. Secure the Signed Authorization Letter to TRASTS CLAST at least one (1) working day upon submission. The DLSMHSI Employee should also provide a receiving copy of the Signed Authorization Letter after they enroll their dependent.
Employee's Dependent Scholarship Program (EDSP) - Grandfather Claus (GFC)

Prerequisite:
Regular / permanent employee / permanent full-time faculty of De La Salle Medical and Health Sciences Institute who have been employed before May 31, 1988.
Application Procedures (for new applicants):
1. Prepare the following requirements:
a. Scholarship Tuition Fee Discount Authorization Form (to be signed and verified by HRD).
b. Prospectus of the course enrolled in (for dependents who will enroll at DLSU-D)
c. A clear copy of the DLSMHSI employee's birth certificate and their dependent / adoption paper if the dependent is legally adopted.
d. Certificate of admission to the school of the course enrolled in (for dependents who will enroll at DLSU-D)

St. La Salle Medical Education Benefit (SLMEB)

Prerequisite:
Regular/permanent employee/permanent full-time faculty of De La Salle Medical and Health Sciences Institute
Application Procedures (for new applicants):
1. Prepare the following requirements:
a. Scholarship Tuition Fee Discount Authorization Form (to be signed and verified by HRD).
b. Prospectus of the course enrolled in (for dependents who will enroll at DLSU-D)
c. Photocopy of the National Medical Admission Test (NMAT) Result-80% Rating and above
2 . Download or secure the following forms from The Registrar – Admissions, Scholarships and Testing Section (TRASTS)
a. Application Form President Scholarship Program (SLMEB)
b. Scholarship Application Form (to be filled out by the dependent)
c. Memorandum of Agreement for SLMEB
d. Authorization Letter for DLSMHSI SLMEB Enrollment
3. Submit the completed and signed forms and requirements to Center for Lasallian Admission, Scholarship and Testing (CLAST) every Monday-Friday, 8:00 am – 5:00 pm, at least two (2) weeks before the enrollment period.
