
Online Application for Doctor of Medicine
PRELIMINARIES
- Fill out the Online Application Form and follow the instructions given.
For DLSMHSI students: - Log in to the Student’s Portal.
- Click “Enrollment”.
- Input the school year you are applying for.
- Select “Doctor of Medicine”.
- Click “Apply for 2nd course”.
- Pay the Admissions Processing Fee of
Php2,000.00 (for Filipino applicants) or $250.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures).
Note: If you haven’t received your Official Receipt from our cashier, you may contact them directly for updates by emailing [email protected] or by calling (6346) 481-8000 (Cavite line) or (632) 8988-3100 local 1433 (Manila line). - Submit the following requirements and information together with the proof of payment (validated deposit slip) to The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) through email at

General Requirements:
- NMAT Results
- Transcript of Records (TOR) / Certification of Grades
- PSA Birth Certificate
- Certificate of General Weighted Average (GWA) / Certificate of General Pointed Average (GPA)
- Two (2) sets of Certification of Recommendation from previous professor
- Notarized Sworn Statement
- Certificate of Graduation / Diploma or Certificate of Candidacy for Graduation
- Completed Google Form
4. After receiving your confirmation email from TRASTS, log in to the applicant’s portal and choose your preferred interview schedule.
5. Please read the written guidelines and other important details provided in preparation for your interview.
- A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their interview (click here for rescheduling procedures).
6. Check the applicant’s portal at least one (1) week after your interview or contact TRASTS for the final result of the evaluation.
CONFIRMATION
1. Secure the Acceptance Letter from TRASTS through the applicant’s portal, which contains the link for complete instructions on the next steps, should the applicant pass all the screening and evaluation criteria.
2. Pay the Php35,000 reservation fee through Dragonpay (see link for the bank details and video for the payment procedures) to confirm your slot.
- The reservation fee is non-refundable and non-transferable but should the applicant enroll in this Institution, the aforementioned amount shall be deducted from their total tuition fee.
- Applicants who do not comply with all the requirements regardless of payment shall have their reservation fees forfeited.
- Failure to comply with the aforementioned policy would mean forfeiture of slots.

- Completed and signed Acknowledgement Letter (for Filipino applicants / for foreign applicants)
- Completed Applicant Personal History Statement
- Completed and signed Declaration of Agreement
- Completed and signed Special Needs Assessment Form
- Completed and signed
Declaration of Citizenship

4. After receiving a confirmation email from TRASTS, log in to the applicant’s portal and choose your medical examination and uniform measurement schedules.
5. Submit the following requirements through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or via mail/courier to receive your Enrollment Permit and Enrollment Procedure in the applicant’s portal:
For Medicine applicants from DLSMHSI
- Original NMAT Results
- Photocopy of Birth Certificate
- Certificate of Graduation or Photocopy of Diploma
- Original Notarized Sworn Statement
- 1 pc. 2×2 picture (white background)
For Medicine applicants from other schools
- Transfer Credential / Honorable Dismissal from previous school
- Original NMAT Results
- Photocopy of Birth Certificate
- Certificate of Graduation or Photocopy of Diploma
- Original Certificate of General Weighted Average (GWA) / Certificate of General Pointed Average (GPA)
- Original Notarized Sworn Statement
- Certificate of Moral Character issued by the Student Affairs and its equivalent
- 1 pc. 2×2 picture (white background)
For applicants who cannot submit the said original documents on or before enrollment, you can submit the Original Notarized Provisional Enrollment Form and Sworn Statement for you to enroll temporarily.
If the lacking requirements become available before the specified date on your Notarized Provisional Enrollment Form, you may submit them to TRASTS through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or through mail/courier at the address below so we can update your enrollment status.
