
Online Application for Doctor of Medicine
PRELIMINARIES
- Fill out the Online Application Form and follow the instructions given.
For DLSMHSI students: - Log in to the Student’s Portal.
- Click “Enrollment”.
- Input the school year you are applying for.
- Select “Doctor of Medicine”.
- Click “Apply for 2nd course”.
- Pay the Admissions Processing Fee of
Php2,000.00 (for Filipino applicants) or $250.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures).
Note: If you haven’t received your Official Receipt from our cashier, you may contact them directly for updates by emailing proofofpayment@dlsmhsi.edu.ph or by calling (6346) 481-8000 (Cavite line) or (632) 8988-3100 local 1433 (Manila line). - Submit the following requirements and information together with the proof of payment (validated deposit slip) to The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) via email at
admissionscm@dlsmhsi.edu.ph, specifying the following:

General Requirements:
- NMAT Results
- Transcript of Records (TOR) / Certification of Grades
- PSA Birth Certificate
- Certificate of General Weighted Average (GWA) / Certificate of General Pointed Average (GPA)
- Two (2) sets of completed and signed Certification of Recommendation from previous professor / previous employer (if the applicant is working already).
- Notarized Sworn Statement
- Certificate of Graduation / Diploma or Certificate of Candidacy for Graduation
- Completed Google Form
4. After receiving your confirmation email from TRASTS, log in to the applicant’s portal and choose your preferred interview schedule.
5. Please read the written guidelines and other important details provided in preparation for your interview.
A rescheduling fee of Php 500.00 shall be paid if the applicant fails to attend or wishes to reschedule their interview (click here for rescheduling procedures).
6. Check the applicant’s portal at least one (1) week after your interview or contact TRASTS for the final result of the evaluation.

Qualified applicants shall receive an Acceptance Letter from TRASTS through the applicant portal with a link to the complete instructions for the next step (Reservation of slot /
Direct Enrollment).

For applicants who cannot submit the said original documents on or before enrollment, you can submit the
Original Notarized Provisional Enrollment Form for you to enroll temporarily.
Applicants from DLSMHSI undergraduate program
1. Original NMAT Results
2. Photocopy of Birth Certificate
3. Photocopy of Transcript of Records
4. Certificate of Graduation or Photocopy of Diploma
5. Original Notarized Sworn Statement
6. 1 pc. 2×2 picture (white background)
Applicants from other schools
1. Transfer Credential / Honorable Dismissal from previous school
2. Original NMAT Results
3. Photocopy of Birth Certificate
4. Certificate of Graduation or Photocopy of Diploma
5. Original Certificate of General Weighted Average (GWA) / Certificate of General Pointed Average (GPA)
6. Original Notarized Sworn Statement
7. Certificate of Moral Character issued by the Student Affairs and its equivalent
8. 1 pc. 2×2 picture (white background)
Additional Requirements for Foreign applicants
1. Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holders of any valid visa and Certificate of Residence for Temporary Students (CRTS) (for foreign applicants).
2. English Proficiency Result (eg, TOEIC / IELTS, etc.) or from the Institute’s Language Training & Professional Development Unit (LTPDU) (for non-native English speakers).
3. Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from the Institute’s Language Training & Professional Development Unit (LTPDU).
Additional Requirements to prepare for Visa Conversion
1. Original passport with a valid tourist visa (at least one (1) month remaining).
2. Two (2) photocopies each of the passport biopage latest arrival stamp, and updated tourist visa.
3. 1 piece of a 2x2 colored picture.
4. Medical Certificate - indicating "Fit to study" (bring all medical exam results to the Industrial Clinic located at MAC Building, 3rd floor (look for Mr. Emjay).
5. A copy of the Drug test result.
6. National Bureau of Investigation (NBI) clearance.
Note: All foreign students are required to pay the
$7,000.00 foreign fee upon enrollment.
