Scholarship APPLICATION
Online scholarship application procedure
- Fill out the Scholarship Application Form.
- Scan or take a clear photo of the form and submit the requirements to The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) via email at [email protected]
Specify the following:

- For DLSMHSI employees, please coordinate with Ms. Wendy Acol thru email: [email protected]
- Other requirements such as the Scholarship Application fee (Php50.00), daily time record, and evaluation (for financial scholars) shall be settled once the regular classes resume.
- Please wait for an email from The Registrar - Admissions, Scholarships and Testing Section (TRASTS) to confirm the submission of your application.
- Wait for a confirmation email from The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) acknowledging the receipt of your application.
- Monitor official channels for updates on the status of your application. TRASTS will release the list of approved tuition fee discounts through student councils, college representatives, individual emails, and official social media platforms. You may also contact Ms. Wendy Acol through the following:

Please read the Scholarship Policies and Guidelines Effective SY 2022-2023.
If your name appears on the list of approved scholars, you are required to submit your signed Memorandum of Agreement (MOA) as soon as possible (except for Entrance Scholarship recipients) via email to
[email protected]. Please include the following information in your email to ensure the processing of your tuition fee discount:

- MOA can be downloaded through the Scholarship page under downloadable forms.
- For EBPSP scholars, tuition fee discount shall be automatically deducted upon enrollment once you submit all required documents before enrollment.
To inquire about updates on your balances/student ledger or request for refund, you can directly coordinate with the DLSMHSI Student Accounts:
