PRELIMINARIES
1. Fill out the Online Application Form and follow the instructions given (for DLSMHSI SHSSHS students, click here for next step).
2. Email the following requirements to emilagan@dlsmhsi.edu.ph:
- SAT Score Result
- Grade 11/12 grades (Any available grades and from the school portal will do)

3. After receiving your confirmation email, pay the non-refundable / non-transferable College Admission Test (CAT) fee of Php 1,000.00 (for Filipino applicants) or $125.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures).
Note: If you haven’t received your Official Receipt from our cashier, you may contact them directly for updates by emailing
proofofpayment@dlsmhsi.edu.ph or by
calling (6346) 481-8000 (Cavite line) or (632) 8988-3100 local 1433 (Manila line).
4. After receiving your Official Receipt from our cashier, log in to the applicant’s portal and choose your preferred interview schedule
(for
BS Nursing, BS Pharmacy, BS Radiologic Technology, BS Nuclear Medicine Technology, and
BS Biochemistry applicants only).
For
Doctor of Dental Medicine, BS Physical Therapy, BS Occupational Therapy, and
BS Speech-Language Pathology applicants, they can directly proceed to the Confirmation procedure.

REQUIREMENTS
Requirements Needed for Enrollment:
For applicants who cannot submit the said original documents on or before enrollment, you can submit the following requirements to enroll temporarily:
- Original Notarized Provisional Enrollment Form
- Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary (for Transferee applicants)
Applicants from DLSMHSI
1. Photocopy of Birth Certificate
2. Photocopy of High School Diploma or Certificate of Graduation
Applicants from Other Schools
1. Photocopy of Birth Certificate
2. Original Form 138 (4th year High School Report Card)
3. Photocopy of High School Diploma or Certificate of Graduation
Transferee Applicants
1. Photocopy of Birth Certificate
2. Original Transfer Credential/Honorable Dismissal from the previous school
3. Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary
Additional Requirements for Foreign Applicants
1. Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native-born, children of diplomats, or holders of any valid visa, and Certificate of Residence for Temporary Students (CRTS).
2. English Proficiency Result (e.g., TOEIC / IELTS, etc.) or from the Institute’s Language Training & Professional Development Unit (LTPDU) (for non-native English speakers).
3. Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from the Institute’s Language Training & Professional Development Unit (LTPDU) (for Nursing applicants).
Additional Requirements to Prepare for Visa Conversion
1. Original passport with a valid tourist visa (at least one (1) month remaining).
2. Two (2) photocopies each of the passport biopage, latest arrival stamp, and updated tourist visa.
3. One (1) piece of a 2x2 colored picture.
4. Medical Certificate – indicating "Fit to study" (bring all medical exam results to the Industrial Clinic located at MAC Building, 3rd floor; look for Mr. Emjay).
5. A copy of the drug test result.
Note: All foreign students are required to pay the $1,000.00 foreign fee upon enrollment.
