Online Application for Undergraduate PROGRAMS
(TRANSFEREE APPLICANTS)
Transferees are applicants who have already started their undergraduate programs in other schools but wish to continue their studies at DLSMHSI.
REMINDERS
1. Prepare the following requirements as preparation for the evaluation/crediting of subjects on or before your enrollment:
a. Transcript of Records (TOR) / Certification of Grades (even from the school portal will do)
b. Subject / Course description of subjects taken from previous school and/or course.
c. Honorable Dismissal / Transfer Credential
2. Acceptance of the Transferee applicants is subject to approval and slot availability.
3. BS Nursing and BS Medical Laboratory Science programs do not accept transferee applicants, unless the applicant is willing to start as a Regular applicant (only NSTP will be credited).
PRELIMINARIES
1. Fill out the

2. Pay the non-refundable / non-transferable College Admission Test (CAT) Fee of
Php 1,000.00 (for Filipino applicants) or
$125.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures).
Note: If you haven’t received your Official Receipt from our cashier, you may contact them directly for updates by emailing
proofofpayment@dlsmhsi.edu.ph or by calling (6346) 481-8000 (Cavite line) or (632) 8988-3100 local 1433 (Manila line).
3. After receiving your Official Receipt from our cashier, log in to the applicant portal and choose your preferred Entrance Examination schedule.
4. Read and watch the written and video guidelines provided and other details for your entrance examination in your portal.
5. After passing the Entrance Exam, all BS Nursing, BS Pharmacy, BS Radiologic Technology, BS Nuclear Medicine Technology, and BS Biochemistry applicants shall log in to their portal to choose and undergo an interview.
For
Doctor of Dental Medicine, BS Physical Therapy, BS Occupational Therapy, and
BS Speech-Language Pathology applicants, they can directly proceed to the confirmation procedure.
Confirmation
Qualified applicants shall receive an Acceptance Letter from TRASTS through the applicant portal with a link to the complete instructions for the next step
(Reservation of slot /
Early Enrollment /
Direct Enrollment).
REQUIREMENTS
Requirements Needed for Enrollment:
For applicants who cannot submit the said original documents on or before enrollment, you can submit the following requirements to enroll temporarily:
- Original Notarized Provisional Enrollment Form
- Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary (for Transferee applicants)
Transferee Applicants
1. Photocopy of Birth Certificate
2. Original Transfer Credential/Honorable Dismissal from the previous school
3. Completed and signed Advising & Crediting Form from the DLSMHSI College Secretary
Additional Requirements for Foreign Applicants
1. Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native-born, children of diplomats, or holders of any valid visa, and Certificate of Residence for Temporary Students (CRTS).
2. English Proficiency Result (e.g., TOEIC / IELTS, etc.) or from the Institute’s Language Training & Professional Development Unit (LTPDU) (for non-native English speakers).
3. Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from the Institute’s Language Training & Professional Development Unit (LTPDU) (for Nursing applicants).
Additional Requirements to Prepare for Visa Conversion
1. Original passport with a valid tourist visa (at least one (1) month remaining).
2. Two (2) photocopies each of the passport biopage, latest arrival stamp, and updated tourist visa.
3. One (1) piece of a 2x2 colored picture.
4. Medical Certificate – indicating "Fit to study" (bring all medical exam results to the Industrial Clinic located at MAC Building, 3rd floor; look for Mr. Emjay).
5. A copy of the drug test result.
Note: All foreign students are required to pay the $1,000.00 foreign fee upon enrollment.
