1. Fill out the Online Application Form and follow the instructions given (for DLSMHSI SHSSHS students, proceed to Screening and Evaluation number 1 or click here).
   
   
2. Pay the non-refundable / non-transferable College Admission Test (CAT) Fee of Php 500.00 (for Filipino applicants) or $125.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures). 
   
   
3. After receiving your Official Receipt from our cashier, log in to the applicant’s portal and choose your preferred Entrance Examination schedule.
   
   Note: The online exam is available for applicants from outside Cavite only. Please send an email to [email protected] to request the online exam. Kindly provide your temporary ID number, full name, and address. Slots are limited and will be allocated on a first-come, first-served basis, subject to approval.

4. Read the written guidelines and watch the video instructions, along with other details provided for your entrance examination.

• Take the "Practice Examination" at your earliest convenience (during your free time) to familiarize yourself with the general aspects of the exam. If you encounter any issues opening the software, TRAST will be available to assist and resolve the problem before your actual exam.

• Take the Online CAT as scheduled. Applicants should be onsite (for face-to-face takers) or logged in to the designated Zoom link (for online takers) at least 30 minutes before the examination. Please note that applicants are expected to have read the Guidelines on Taking the DLSMHSI Online Entrance Examination and have already completed the Practice Exam.

• A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their entrance examination (click here for rescheduling procedures).

• The result of the Online CAT shall be released through the applicant’s portal after finishing the entrance examination.

2. After you receive your Official Receipt from our cashier, log-in to your applicant’s portal and choose your preferred interview schedule.
   
   Note: For applicants to BS Biochemistry, BS Nursing, BS Radiologic Technology, and BS Nuclear Medicine Technology, kindly send a message to our Facebook account (DLSMHSI Interview) to schedule an online interview. Please include your full name, ID number, and the program you are applying for. Kindly note that only applicants residing outside Cavite are eligible to request an online interview.
   
   
3. Please read the written guidelines and other important details provided in preparation for your interview.
   
   A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their interview schedule.
   
   
4. Check the applicant’s portal 1-2 days after your interview or contact The Registrar - Admissions, Scholarships and Testing Section (TRASTS) for the final result of the evaluation.

Secure the Acceptance Letter from TRASTS through the applicant’s portal, which contains the link for complete instructions on the next steps, should the applicant pass all the screening and evaluation criteria.
