Online Application for Doctor of Philosophy in Health Sciences by Research

Applicants whose PhD dissertations fall within the following broad areas are eligible for admission to the PhD in Health Sciences program:

Nursing

Public/Community Health

Epidemiology

Health Science/Medical Education

Pharmacy

Rehabilitation Sciences

SCREENING AND EVALUATION

  1. Fill out the  Online Application Form  and follow the instructions given.
     For DLSMHSI students:
  2. Log in to the Student’s Portal. 
  3. Click “Enrollment”.
  4. Input the school year you are applying for.
  5. Select “Doctor of Philosophy in Health Sciences”.
  6. Click “Apply for 2nd course”.

  7. Pay the Admissions Processing Fee of Php2,000.00(for Filipino applicants) or $250.00 (for foreign applicants) through Dragonpay (see link for the bank details and video for the payment procedures). 

  8. Submit the following requirements and information together with the proof of payment (validated deposit slip) to The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) through email at [email protected], specifying the following:

General Requirements:

  • Letter of Intent to enroll PhD in Health Sciences, addressed to the Program Director of PhD in Health Sciences program (Dr. Margel C. Bonifacio)
  • Curriculum Vitae
  • Master’s Diploma
  • Master’s Transcript of Records
  • Completed and Signed Preliminary Information for Application
  • Completed Capsule Proposal for Dissertation 1 & 2 


4. After receiving your confirmation email from TRASTS, log in to the applicant’s portal and choose your preferred interview schedule.

5. Please read the written guidelines and other important details provided in preparation for your interview.



  • A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their interview schedule.

6. Submit the following documents immediately (after the interview) to [email protected]. Label your email subject as "Additional Documentations for PhD Admission”.

  1. Documentations of research engagements (submit all available):
  2. Experience in teaching research (certification)
  3. Research project outputs (abstract)
  4. Resource speaker/person in research (certificates)
  5. Attendance to research conferences/workshops (certificates)
  6. Master’s thesis (abstract)
  7. Publications (full text)
  8. Other related documentations
  9. For foreign applicants: English Proficiency Certification (Reading, Writing, Speaking and Listening). Applicants may have the option to take TOEIC at DLSMHSI.

7. Check the applicant’s portal at least one (1) week after your interview or contact TRASTS for the final result of the evaluation.

Confirmation

  1. Secure the Acceptance Letter from TRASTS through the applicant’s portal, which contains the link for complete instructions on the next steps, should the applicant pass all the screening and evaluation criteria.

  2. Pay the Php10,000 reservation fee through Dragonpay (see link for the bank details and video for the payment procedures) to confirm your slot. 
  3. The reservation fee is non-refundable and non-transferable but should the applicant enroll in this Institution, the aforementioned amount shall be deducted from their total tuition fee
  4. Applicants who do not comply with all the requirements regardless of payment shall have their reservation fees forfeited. 
  5. Failure to comply with the aforementioned policy would mean forfeiture of slots

4. Submit the following requirements through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or via mail/courier to receive your Enrollment Permit and Enrollment Procedure in the applicant’s portal:


Requirements:


  • Transfer credential / Honorable dismissal from previous school
  • Original PSA Birth Certificate


Additional requirements for foreign applicants:

  • Photocopy of passport
  • Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid visa and Certificate of Residence for Temporary Students (CRTS) 
  • Special Study Permit (SSP) for foreign students studying in the Philippines for less than a year or are less than 18 years old

For applicants who cannot submit the said original documents on or before enrollment, you can submit the Original Notarized Provisional Enrollment Form and Sworn Statement for you to enroll temporarily. 

If the lacking requirements become available before the specified date on your Notarized Provisional Enrollment Form, you may submit them to TRASTS through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or through mail/courier at the address below so we can update your enrollment status.
