Admission Policies, Terms, and Applicant's Disclaimer

General Policies (Applicable to All Applicants)

1.  Application and Other Fees

• All application, testing, evaluation, and processing fees are **non-refundable and non-transferable**, regardless of the outcome of the application or if the applicant decides not to continue the admission process.

• Applicants are advised to verify that their intended program is still accepting applications before making any payment.

2.  Submission of Information and Documents

• The applicant certifies that all information and supporting documents submitted are true, complete, authentic, and accurate.

• The Institution reserves the right to verify all submitted documents with the issuing schools, agencies, or organizations.

• Submission of fraudulent, altered, or misleading information or documents shall be grounds for the denial of admission, cancellation of admission, forfeiture of reservation, or dismissal from the Institution if discovered after enrollment.

3.  Admission is Not Guaranteed

• Submission of an application does not guarantee admission.

• Admission is subject to the applicant's compliance with all admission requirements, entrance examination (if applicable), interview, medical examination, academic evaluation, availability of slots, and approval by the concerned academic unit.

4.  Program Availability

• The Institution reserves the right to suspend, close, or discontinue the acceptance of applications for any academic program once the maximum number of qualified applicants has been reached or when deemed necessary.

5.  Reservation of Slot

• Admission slots are limited and shall be offered on a first-qualified, first-served basis.

• Applicants are encouraged to reserve their slot within the prescribed period to secure their admission. Failure to reserve within the given deadline may result in the release of the slot to other qualified applicants.

6.  Compliance with Institutional Policies

• Applicants agree to abide by all admission policies, academic regulations, institutional rules, and future policy updates implemented by the Institution.

7.  Data Privacy

• By submitting an application, the applicant authorizes the Institution to collect, process, store, and use personal information in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and the Institution's Data Privacy Policy for legitimate admission, enrollment, scholarship, and  academic purposes.

Additional Policies for Transferee Applicants

1.  Application Period

• Entrance examination and subject evaluation for all transferee applicants shall be conducted beginning in July and onwards, subject to the prescribed admission schedule and availability of evaluation slots.

• Admission slots for transferee applicants are not guaranteed. Regular applicants are given priority in the allocation of available slots, and transferee applicants may only be accommodated if slots remain available in their intended program.

• Completion of the entrance examination, subject evaluation, and other admission requirements does not guarantee admission or enrollment.

        Final admission is subject to the applicant's qualification, results of the evaluation,

        availability of slots, and approval of the concerned academic unit.

2.  Subject Crediting

• Only General Education (GenEd) subjects may be credited, and those that will satisfy the Institution's curriculum requirements and academic policies

        may be considered for credit.

• The final decision on the crediting of subjects rests solely with the concerned College, School, or Academic Unit after the official evaluation of credentials.

3.  Reservation of Slot

• Accepted transferee applicants are strongly encouraged to reserve their admission slot immediately upon acceptance since transferee enrollment is conducted after the regular enrollment period; failure to reserve a slot may result in the forfeiture of admission due to limited program capacity.

Medical Examination PolicieS

1. Applicants shall report only on their confirmed medical examination schedule.

2.  If classes or office operations in Cavite are suspended due to inclement weather, natural calamities, government announcements, or other unforeseen circumstances, all scheduled medical examinations for that day shall automatically be cancelled.

3.  Applicants affected by the cancellation will receive further instructions through email or may log in to the Applicant Portal to select a new medical examination schedule after at least one (1) working day.

No rescheduling fee shall be charged for medical examinations cancelled due to official class or office suspensions.

4.  Applicants who fail to attend their confirmed medical examination schedule without prior notice or a valid reason may be required to pay the applicable rescheduling fee, subject to existing institutional policies.

5.  Accepted applicants and/or enrolled students who are not medically cleared following the required medical examination may be disqualified from admission or enrollment. For enrolled students, a Leave of Absence (LOA) shall be filed in accordance with the applicable institutional policies.

Any applicable reservation or enrollment fees paid by the applicant/student shall be processed for refund, subject to the Institute's refund policies and procedures.

Applicants/students who subsequently obtain the required medical clearance may reapply for admission and/or enrollment in the next School Year,

subject to the admission requirements, available slots, and other applicable policies of the Institute.

The medical clearance shall be determined by the authorized medical personnel of DLSMHSI based on the results of the required medical examination and

any additional evaluation deemed necessary.

Applicant's AcknowledgmenT

By proceeding with the online application, I acknowledge that I have read, understood, and agreed to the foregoing policies, terms, and conditions.

I certify that all information and documents I have submitted are true, complete, and authentic, and I understand that any violation of these policies may

affect my admission to the Institution.

• Incomplete Applications – Applications with incomplete requirements will not be processed until all required documents are submitted.

• Communication Responsibility – Applicants are responsible for regularly checking their Applicant Portal and registered email for announcements, schedules, and admission updates.

• Missed Schedules – Failure to appear for scheduled examinations, interviews, or medical examinations without prior notice may require rescheduling and payment of applicable fees.

• Scholarship Disclaimer – Admission does not automatically qualify an applicant for scholarships or financial assistance. Scholarship applications are evaluated separately and are subject to their respective eligibility requirements.

• Reservation Fee Policy – Reservation fees are non-refundable and non-transferable and may only be applied to the applicant's own enrollment in the approved term, subject to institutional policy.

• Right to Modify Policies – The Institution reserves the right to amend admission policies, schedules, fees, and procedures without prior notice when necessary.

• Official Communication – Only announcements sent through the official admissions office, the Applicant Portal, or the Institution's official communication channels shall be considered valid. Information from unofficial sources shall not be binding.

• Application or any fees paid are non-refundable and non-transferable.

• Please make sure first if your program applying is still open before you pay the necessary fees.

For Transferee applicants

• Entrance exam and subject evaluation are conducted only by month of July. Slots are subject to availability.

• Only General Education subjects may be credited by the College (subject for final approval during subject evaluation).

• Accepted Transferee applicants are highly recommended to reserve their slots, since enrollment for transferee are held later compare to the regular applicants.

During Medical Examination

• If there is a class suspension in Cavite (e.g. because of the typhoon, government announcements, etc.), the Medical Examination is automatically cancelled. Kindly wait for our email or visit your applicant’s portal again to choose another medical examination schedule (no need to pay the rescheduling fee)

        after at least one (1) working day.
